New Patient Doctor`s Report

We set aside special appointment times to review findings and recommendations for new patients.  This is the most vital appointment you will have in our office!  You will learn a tremendous amount of information, so we suggest that you bring your spouse, friend, or family member along to help you remember.  We will also be reviewing the changes that you need to make at home and work.
